Steps:
- Read the Inclusion/Exclusion Criteria under Policies to make sure your service qualifies to be listed.
- Complete the Intake Form entitled: Overall Agency Information
- Complete the applicable Services Intake Forms
- Please repeat this process annually (or sooner, if your information changes)
- 10 – 30 minutes of your time to complete the necessary forms, depending on the variety of services you offer.
- Approximately 10 days for our staff to review this information and enter it into the Search for Services database.
